Viral hepatitis B, C, and delta in the general adult population living in Mayotte: the 2018–2019 Unono Wa Maore Survey
Objectives - To estimate, among the general population aged 15–69 living in Mayotte, the prevalence of hepatitis B (HBV), C (HCV), and delta (HDV) virus infections; to determine the proportion of people not immunized against HBV; and to describe the epidemiological and virological characteristics of infected individuals. Methods - The Unono Wa Maore survey, conducted in 2018–2019 as a household-based study among a random sample of the general population living in Mayotte, included the collection of epidemiological data and a venous blood sample. Serological and molecular markers for HBV, HCV, and HDV were tested for. Results - Among 5,207 individuals aged 15–69 who were contacted, 4,643 completed the questionnaire (89.2%), of whom 2,917 were tested (62.8%). Seventy-six individuals tested positive for the HBs antigen (Ag), representing an estimated prevalence of HBV infection of 3.0% (95% CI: [2.3–3.9]). One individual tested positive for HDV antibodies (Ab) (0.65%), with a negative HDV RNA viral load. A past HBV infection that had resolved was found in 27.8%. The proportion of people immunized by vaccination (HBsAb = 10 mIU/mL), estimated at 27.7% for those aged 15–69, was highest among those aged 15–29 (37.9%). However, 47.9% of those aged 15–29 were not immune (either through vaccination or past infection). Six individuals tested positive for HCV antibodies (0.21%), three of whom had detectable HCV RNA. Conclusion - These results confirm that Mayotte is an area of low endemicity for HCV and HDV, but of moderate endemicity for HBV. With an HBsAg prevalence ten times higher than in mainland France and a high proportion of unimmunized 15- to 29-year-olds, hepatitis B must be a public health priority in Mayotte.
