Interim Evaluation of START-HPV, a pilot program for the primary screening of precancerous and cancerous cervical lesions using HPV testing in the Ardennes department, France, 2012–2013.
This article presents an interim report on the results of the START-HPV pilot program for the screening of precancerous and cancerous cervical lesions. The objective of this program is to evaluate the practical conditions for implementing primary screening using HPV testing. The eligible population consists of 32,444 women residing in the Ardennes region, aged 31 to 65, who have not had a Pap smear in the past 3 years and who are invited to undergo a cervical cell sample (PCU) for screening analyzed by HPV testing. Following the initial invitation and first follow-up phases, the provisional participation rate stands at 6.2%. The cervical smears were performed by general practitioners (50.6%), gynecologists (46.6%), or other healthcare professionals (2.8%). The rate of non-compliant PCUs is 2.2%. The rate of positive HPV tests is 6.4%, with the average age of the participating population being 51 years. Among HPV-positive women, follow-up cytology reveals abnormalities in 43.3% of cases. The second follow-up phase, which includes an offer of self-sampling, is still ongoing and will conclude in 2014. Despite an ongoing enrollment phase, participation is low in this population, despite significant involvement from general practitioners. This likely reflects the difficulty of implementing a new screening program. The final results of the START-HPV pilot study will be available in late 2014. (R.A.)
