Hospital morbidity associated with Alzheimer's disease and related conditions based on data from the Program for the Medicalization of Information Systems (PMSI), France 2007

Introduction. In 2001, it was estimated that 24.3 million people worldwide had Alzheimer’s disease (AD) or related disorders, with 60% of them living in developed countries. The only existing prevalence data in France come from cohort studies and do not allow for measuring changes in the prevalence of these diseases. The use of the PMSI would help partially address this gap. The objective of this study is therefore to provide national-level hospital morbidity indicators that will enable monitoring of the disease’s progression. Methods. The data are drawn from the national MCO (short-stay) database of the 2007 PMSI. The selected and subsequently linked hospital stays correspond to admissions lasting 24 hours or more, or resulting in death, associated (DP, DA, and/or DR) with Alzheimer’s disease (F00, G30) or a related form of dementia (F01, F03). Results. In 2007, 163,125 people were hospitalized in connection with Alzheimer’s disease or dementia for a total of 212,308 hospital stays. Nearly 80% of patients had only one hospital stay during the year. The median length of stay was five days. Women outnumbered men by a factor of two, and 70% of patients were 80 years of age or older. Nearly 10% of patients (n = 16,195) died as a result of their hospitalization. The standardized rate (French population, 2006) of hospitalized patients was 253.0 per 100,000 person-years. Discussion and Conclusion. The sex ratio, age groups, and increases in age-specific rates are consistent with cohort estimates. However, this is not the case for the overall rate. One hypothesis is that hospitalized patients likely have advanced AD. (R.A.)
