High Blood Pressure in France: Prevalence, Treatment, and Control in 2015 and Trends Since 2006
Introduction: Hypertension is the most common chronic condition in France, affecting nearly one in three adults. It is a major risk factor for cardiovascular and cerebrovascular diseases. One of the objectives of the Esteban study was to estimate the prevalence of HTN in France, its screening, and its management in 2015, and to examine trends since the 2006 National Nutrition and Health Survey (ENNS). Methods: The data come from the national Esteban survey, a cross-sectional study conducted in France between 2014 and 2016 among adults aged 18 to 74. This study included a face-to-face questionnaire survey, a self-administered questionnaire, a dietary survey, and a health examination. Blood pressure (BP) was measured during the health examination, conducted at a health screening center or at home. Hypertension was defined as systolic blood pressure (SBP) ≥140 mmHg and/or diastolic blood pressure (DBP) ≥90 mmHg, or reimbursement for at least one antihypertensive medication. Results: A total of 2,169 adults had at least two blood pressure measurements, including 974 men (45%) and 1,197 women (55%). The prevalence of HTN was 30.6% [95% CI: 28.1–33.2]. The prevalence of hypertension was higher among men than among women (36.5% vs. 25.2%) and increased with age. Only 1 in 2 people were aware of their hypertension. Among those with hypertension, 47.3% [45.1–54.8] were being treated with an antihypertensive medication. Among those receiving treatment, only 55.0% had controlled blood pressure (44.9% among men and 66.5% among women). Since 2006, while the prevalence, awareness, and control of this condition have remained stable, the proportion of women receiving treatment has decreased significantly (p=0.008). Conclusion: Since 2006, no decrease in the prevalence of hypertension has been observed in France, with one in three adults still having hypertension. Furthermore, no improvement in the screening and management of hypertension has been demonstrated. Among women, therapeutic management has even deteriorated over the period.
