Stroke in France: Patients Hospitalized for Stroke in 2014 and Trends from 2008 to 2014
Introduction: In France, stroke is the leading cause of death among women and the third leading cause among men. Hospitalization rates for stroke increased among people under 65 between 2002 and 2008. Following the implementation of the National Stroke Action Plan (2010–2014), this study analyzes trends in hospitalization rates and care in neurovascular units (NVUs) since 2008. Methods: Data were obtained from the national hospitalization database of the Program for the Medicalization of Information Systems in Medicine, Surgery, and Obstetrics (PMSI-MCO, 2008–2014). For each patient, the first hospitalization of the year for stroke was selected. The crude rates of hospitalizations and in-hospital mortality by stroke type, age group, and sex were calculated and then age-standardized. Temporal trends were analyzed using Poisson regression. Results: In 2014, 110,438 patients were hospitalized for stroke. The proportion of patients admitted to the stroke unit reached 51.5% for ischemic strokes and 33.2% for intracerebral hemorrhages. From 2008 to 2014, the age-standardized rate of patients hospitalized for ischemic stroke increased by 14.3% among those under 65 years of age and decreased by 2.1% among those 65 years of age and older. In contrast, for hemorrhagic strokes, the rate changed little (+2.0%). In-hospital mortality from ischemic strokes decreased by 12.5% over the same period. Significant regional disparities in hospitalization rates were observed. Conclusion: The encouraging increase in the proportion of patients hospitalized in the ICU and the decline in in-hospital mortality should not obscure three concerning observations: the stability of the overall rate of patients hospitalized for stroke in the context of an aging population, the increase in the rates of young adults hospitalized for ischemic stroke, and the existence of significant regional disparities in the rates of patients hospitalized for stroke.
