Impact of the AVC Plan on the Management of Established Ischemic Strokes: Trends in the French National Health Authority’s Evaluation Indicators from 2011 to 2016
Introduction: A major component of the 2010–2014 National Stroke Plan was aimed at improving acute care for patients with acute ischemic stroke (AIS). As part of its follow-up, four national evaluation campaigns were coordinated by the Haute Autorité de Santé. Method: The records analyzed were identified via the PMSI-MCO (Program for the Medicalization of Information Systems in Medicine, Surgery, and Obstetrics) in 2011, 2012, 2014, and 2016, with each eligible facility having a maximum of 80 records to analyze (sample selected by random drawing). Results: A total of 87,365 AIC patient records were analyzed. The median time (MT) between symptom onset and arrival at the hospital remained stable between 2011 and 2016 (3 hours 7 minutes in 2016 vs. 3 hours 3 minutes in 2011, p=0.013). The MD from hospital arrival to imaging was significantly reduced (1 hour 42 minutes in 2016 vs. 1 hour 54 minutes in 2011, p<0.001). The thrombolysis rate increased significantly (8.6% in 2011 vs. 14.3% in 2016, p<0.001). Conclusion: With the exception of the time from symptom onset to hospital arrival, this study highlights an improvement in the quality of acute care for ischemic stroke patients, consistent with and concurrent to the organizational restructuring carried out under the National Stroke Plan.
