Hospitalizations in specialized follow-up and rehabilitation care for nervous system disorders in 2017

Introduction - The objectives of this study are to describe the patient population in the specialized follow-up and rehabilitation (SSR) sector for nervous system disorders in 2017: the main conditions treated, factors associated with care in these specialized SSR facilities, regional disparities, and changes that occurred between 2013 and 2017. Methods - Data were drawn from the 2017 PMSI-SSR national databases. Admissions were selected based on the type of medical unit and the major category (CM 01). Regional bed capacity was calculated using the annual statistics on healthcare facilities (SAE). Comparative indices of admission rates to specialized SSR were calculated using indirect standardization by age and region of residence, to enable direct comparison of each regional rate to a value of 100, representing the national average. Results - Specialized SSR for nervous system disorders accounts for 55% of SSR stays for nervous system disorders. The most common conditions are strokes (32% of patients), neurodegenerative diseases excluding dementia (18%), “other nervous system disorders,” which include numerous neurological sequelae (15%), and spinal cord injuries (12%). Between 2013 and 2017, the capacity for beds or beds in "nervous system" SSR facilities increased by 25%. At the same time, admissions increased by 30% and length of stay by 19%. Significant geographic disparities persist: regional comparative indices of hospitalization rates in "nervous system" SSR units range from 75 to 147 in mainland France, with the extent of these disparities being more pronounced in overseas departments and regions. Discussion and Conclusion - This descriptive study on the management of neurological conditions in specialized SSR units for nervous system disorders contributes to the current state of the SSR hospitalization field. It highlights the significant burden of strokes, neurodegenerative diseases, and spinal cord injuries in this specialized sector. Despite the notable increase in capacity in this sector between 2013 and 2017, significant disparities in regional utilization rates persist.
