Screening and Diagnosis of Hepatitis B and C at CeGIDD in 2022, SurCeGIDD Surveillance

Introduction – This article analyzes hepatitis B and C screening and diagnostic activities at the free centers for information, screening, and diagnosis of human immunodeficiency virus (HIV), viral hepatitis, and sexually transmitted infections (STIs) (CeGIDD) for the year 2022. Method – The SurCeGIDD surveillance system relies on the secure transmission of individual data regarding CeGIDD clients to Santé publique France. Screening tests and diagnoses for hepatitis B and C were analyzed according to sociodemographic characteristics and sexual behaviors. The proportions of clients screened and the positivity rates were described for each pathogen and by CeGIDD region. Results – In 2022, 529,453 consultations were reported by 76% of the 304 CeGIDDs surveyed, representing a significant increase in participation rates (62.4% in 2021, 50.3% in 2020, 45% in 2018). The majority of those seeking consultation were cisgender men (64%) and young people under 30 (60%). A total of 141,989 HBsAg and 141,718 anti-HCV antibody serology tests were analyzed. The positivity rates were 1.1% for HBsAg and 0.7% for anti-HCV antibodies (of which 12% were positive for viral RNA). These rates were higher among people born abroad (4.0% for HBV and 1.6% for HCV), cisgender heterosexual men (0.8% for HBV), and transgender people (1.4% for HBV and 1.4% for HCV), as well as cisgender men who have sex with men (MSM) (0.5%) for HCV. Positivity rates were higher among clients in French Guiana, Auvergne-Rhône-Alpes, and Île-de-France for HBV, in Grand Est for HCV, and in Centre-Val de Loire for both viruses. Conclusion – Thanks to increasing participation rates among CeGIDDs, the SurCeGIDD surveillance system allows for a better characterization of hepatitis B and C screening activity and the profile of individuals testing positive. It is therefore important to maintain this participation at a high level and to improve the comprehensiveness and completeness of the data collected.
