A combined analysis of the four indicators of testicular dysgenesis syndrome in France, in the context of exposure to endocrine disruptors: cryptorchidism, hypospadias, testicular cancer, and sperm quality

Introduction: Testicular dysgenesis syndrome (TDS) is a disorder of testicular development that can lead to congenital malformations (hypospadias, cryptorchidism) in boys, an increased risk of testicular cancer, and poor sperm quality in adulthood—the four components of the syndrome. It may be promoted by early exposure to endocrine disruptors (EDs) and has been identified by an international panel of scientists as one of the key indicators to monitor in the context of ED exposure. Methods: We analyzed the temporal and geographic variations of four components of TDS. Epidemiological indicators related to these components were constructed using existing national databases (the Medical Information Systems Program [PMSI] and the National In Vitro Fertilization Registry of the Fivnat association) and studied over comparable time periods to discuss the role of environmental exposure to EDCs. Results: The temporal variations of the four indicators are convergent. They show, over the study periods, a marked deterioration, with no clear trend for hypospadias. In contrast, the geographical variations are generally non-convergent. Conclusions: The results reflect an overall decline in male reproductive health in France, consistent with the international literature. They are consistent with environmental or lifestyle changes, including increasing exposure to EDCs in the general population. At this stage, they do not support a potential role for geographically specific exposures.
