Rise in Risk-Taking Behavior and STDs Among Gay Men: Preliminary Results of the 2000 Gay Press Survey

Conducted among 4,753 respondents, the 2000 Gay Press Survey aimed to assess the state of HIV prevention among gay men living in France. Although most respondents continue to practice effective prevention, very clear changes—the first observed since 1985—have emerged in preventive behaviors. Compared to the situation observed in the 1997 survey, condom use during intercourse was less frequent in 2000 among couples, particularly those who were both HIV-negative and, furthermore, those with mixed HIV status. Furthermore, the decline in preventive behavior is most pronounced among casual partners. Although evident throughout the country and across many groups, the relaxation of preventive measures with casual partners appears most significant among residents of the Île-de-France region, among young people, those with multiple partners, and, even more so, among HIV-positive gay men. Clear trends are also observed in the frequency of risk-taking: between 1997 and 2000, reports of isolated instances of unprotected penetration changed little, unlike reports of repeated (or even regular) exposure to risk. Some gay men appear to have gradually become accustomed to repeated risk exposure and, over time, shifted from occasional (or accidental) risk-taking to more frequent exposure. This trend toward the normalization of unprotected sexual behavior could have significant consequences for the dynamics of the HIV/AIDS epidemic, as suggested by the resurgence of STIs also observed. Factors such as young age, a high number of partners, HIV-positive status, etc., make risk management more complex, but other influences must also be considered. Depression is common among gay men. In recent years, gay culture and sexual practices have changed, as have their perceptions of the risk of HIV transmission due to new treatments. Insights into these various changes allow for the formulation of strategies to guide prevention efforts. (R.A.)
