Geographical variations in deaths from cancers for which screening is available in metropolitan areas, France, 1998–2002.
Introduction - As part of the government’s cancer plan, the InVS has strengthened regional cancer surveillance to inform decision-making and monitor local measures by analyzing mortality data. Geographic variations in deaths from cancers amenable to screening (breast, prostate, colorectal, cervical, and skin melanoma) were studied in this context. Methods - The data analyzed pertain to cancer deaths reported by Inserm’s CépiDc for the period 1998–2002. The indicators used are all-age comparative mortality rates, the proportion of deaths before age 65 among all-age mortality, and comparative rates of potential years of life lost (PYLL) before age 65 by region. Results - Regional differences in all-age mortality are significant, generally more pronounced among men than among women. The greatest variations are observed for prostate cancer (ranging from 8.9 per 100,000 person-years in Corsica to 17.9 in Brittany), while the smallest variations are seen for breast cancer (ranging from 16.4 in Alsace and Auvergne to 24.4 in Nord-Pas-de-Calais). These differences are also significant for deaths before age 65 (30% of all-age deaths among women, 15% among men), which could be partially prevented through early diagnosis and treatment. Regional differences in APVP rates are also significant. They are significantly higher than the national average in Nord-Pas-de-Calais for breast, cervical, and colorectal cancers among women; in Upper Normandy for breast cancer; and in Brittany for prostate cancer. Discussion-Conclusion - This analysis should enable local decision-makers to assess their region’s position relative to the national average and other metropolitan regions, to discuss this situation with local stakeholders who have a good understanding of the local context, and then to implement appropriate measures. More broadly, this analysis aims to inform them of the availability of regional indicators for all cancers on the InVS website, with a view to their use in decision-making. (R.A.)
