First multicenter study of antifungal use in healthcare facilities

Introduction and Objectives: To study the use of antifungal agents (AFAs) in French healthcare facilities in 2012. Materials and methods: The quantities of AFs (WHO class J02, DDD/ATC system) dispensed by the pharmacy in intensive care units, hematology units, and all inpatient units were collected from volunteer HCFs participating in the ATB-RAISIN network in 2012. Data were expressed as the number of defined daily doses (DDD) per 1,000 patient-days (PD). Results: In the 239 participating hospitals, overall ATF consumption was 18 DDD/1,000 HD. It was 163 and 340 DDD/1,000 HD, respectively, in the 97 participating intensive care units and the 26 participating hematology units. Fluconazole accounted for 63% of antifungals in the study as a whole (74% in intensive care, 32% in hematology), amphotericin B for 13%, and voriconazole and caspofungin for 8% each. A wide variety of antifungals was used in hematology, with a predominance of fluconazole and amphotericin B (primarily in liposomal form), and caspofungin, voriconazole, and posaconazole accounting for more than 10% of DAU. In intensive care, caspofungin was the second most commonly used antifungal (12%); the usage profile varied by type of healthcare facility, with higher use of amphotericin B in university hospitals. Conclusion: This study provided the first description of the consumption of all systemic antifungals (not just those on the supplementary list) across a wide range of intensive care and hematology units. Monitoring consumption may help assess the impact of the rational use initiatives implemented. The relationship between consumption and changes in the fungal ecosystem remains to be explored. (R.A.)
