Trends in the incidence rate of lower-limb amputations among people with diabetes and the three-year outcomes of people with diabetes who have undergone amputation, based on PMSI data. ADELF-EMOIS Conference, Bordeaux, April 22–23, 2010
Introduction.—While lower limb amputation (LLA) is a serious complication of diabetes, the long-term outcomes of amputees have been little studied in France. The objective of this analysis is to estimate trends in the LLA rate from 2004 to 2007 and to examine the three-year outcomes of people with diabetes who underwent lower limb amputation in 2004, based on data from the PMSI database. Method. Lower limb amputation procedures were extracted from the PMSI database for mainland France from 2004 to 2007. A diagnosis of diabetes was established when at least one hospital stay (for amputation or other reasons) included a diagnosis of diabetes during this period. Three-year follow-up was conducted on the 8,002 individuals who underwent amputation in 2004 and had a valid anonymization number. Results. In 2004 and 2007, 10,265 and 11,442 AMI events were recorded, respectively, among 8,470 and 9,236 people with diabetes, while the incidence rate of AMI remained stable over this period, at around 375 per 100,000 people with diabetes. In 2004, these amputations consisted of toe amputations (44%), foot amputations (19%), leg amputations (19%), and thigh amputations (18%). During that same year, 12% of patients died in the hospital and 18% underwent reamputation. After three years of follow-up, the cumulative rate of in-hospital death or reamputation was 50%. Discussion/conclusion. The three-year rate of in-hospital death or reamputation is extremely high, reflecting the severity of this complication of diabetes. This rate is nevertheless underestimated because it does not include deaths occurring at home (data not available) or those occurring in post-acute care and rehabilitation facilities (which will be investigated).
