Hospitalizations for foot complications among people with diabetes in France: trends from 2012 to 2022 and the use of preventive measures

Introduction – The objective of our study is to examine trends in hospitalizations for diabetic foot ulcers (DFU) and lower limb amputations (LLA) from 2012 to 2022, socioeconomic and geographic disparities, as well as the use of associated preventive measures.Methods – Data were obtained from the National Health Data System and the 3rd edition of the “National Representative Sample of People with Diabetes” (Entred 3) study. Individuals with diabetes were identified based on reimbursements for antidiabetic treatments. Hospitalizations for PPd were identified using the International Classification of Diseases, 10th Revision, and AMIs using codes from the Common Classification of Medical Procedures. Incidence rates were standardized to the 2010 age structure of the European population among individuals aged 45 and older. For Entred 3, estimates were weighted to account for the sampling design and biases associated with non-participation in questionnaire surveys.Results – Between 2012 and 2022, the incidence of hospitalizations for PPd increased in France (from 692 to 1,006 per 100,000 among men and from 452 to 538 per 100,000 among women). The incidence of AMI, however, remained relatively stable. Socioeconomic gradients are observed, as well as marked regional disparities, with higher rates in the overseas regions and in the Hauts-de-France region. The use of preventive measures remains insufficient, whether it involves foot sensitivity testing, podiatric risk assessment, or the use of podiatry care.Conclusion – Between 2012 and 2022, the rate of hospitalizations for PPd increased, while the rate of AMI remained stable. Prevention efforts must be strengthened, particularly among those who are most socio-economically vulnerable.
