Observation of clinical practices: catheter insertion, catheter and line management, and dressing changes for central catheters. SPIADI National Initiative. Results of the OBSERVA4 survey conducted in 2024
A total of 309 healthcare facilities participated in the 5th OBSERVA4 practice assessment campaign. A total of 7,324 observations were documented, primarily concerning care provided to adult patients (98%). The National Strategy indicators show that compliance rates continue to improve this year, particularly regarding the use of products for skin preparation during catheter insertion and for disinfecting connectors during line management, as well as hand hygiene compliance. These results are very encouraging. Compliance rates are satisfactory for the preventive measures studied, with the exception of those concerning: (1) hand hygiene among healthcare providers, such as surgical hand disinfection using hydroalcoholic rub for central venous catheter placement, the two required hand rubs for short-term central venous catheter placement, and the use of sterile gloves for proximal manipulation of central venous catheters; (2) skin cleansing in children prior to central venous catheter placement and during central venous catheter dressing changes in adults, and (3) valve disinfection during line manipulations. For healthcare professionals performing proximal line manipulations associated with a central line, the compliance rate for wearing surgical masks has decreased, in line with the lifting of the mask mandate following the COVID-19 pandemic. These results help define priorities for action by 2025 and should encourage frontline professionals to use the SPIADI educational tools, which were developed to address the identified training needs.
